New Patient Forms
Complete every form in your packet, including all pages of each application. If a form does not open or you need assistance, call 256-325-1011.
Adult Patients
Please complete all four forms in the adult patient packet.
Pediatric Patients
A parent or legal guardian should complete all five forms in the pediatric patient packet.
Before your child’s appointment: Please ensure the practice has proof of immunizations.
Although we respect every parent’s right to choose what is best for their child, for the safety and consideration of our patients, (we do not accept non-vaccinated patients).
Weight Management
Interested in our medically supervised weight management program? Complete the separate application to provide the information our team needs to review your request.
